چکیده

THE FOOD AND DRUG ADMINIStration (FDA) approval in June 1996 of intravenous recombinant tissue-type plasminogen activator (rt-PA) for patients with acute ischemic stroke treated within 3 hours of symptom onset marked a historic first step in treating this devastating disease. This approval was primarily based on the results of the National Institute of Neurologic Disorders (NINDS) trials (1 and 2). In the NINDS trials, patients with ischemic stroke were treated within 3 hours of symptom onset with either 0.9 mg/kg of rt-PA (alteplase) (maximum dose ,90 mg) or placebo. A significant 11% to 15% absolute benefit was found favoring rt-PA despite a significant increase in the symptomatic intracerebral hemorrhage (ICH) rate (6.4% vs 0.6%; P,.001) at 36 hours. The current approved use of rt-PA is limited to otherwise eligible patients in whom treatment can be initiated within 3 hours after the onset of stroke symptoms. This greatly restricts the number of patients who could potentially be treated, because most stroke patients present more than 3 hours after sympAuthor Affiliations: Oregon Stroke Center, Portland (Dr Clark); Stucky Research Center, Fort Wayne, Ind (Dr Wissman); Stanford Stroke Center, Stanford University, Palo Alto, Calif (Dr Albers); Department of Neurology, University of Alberta, Edmonton (Dr Jhamandas); Marshfield Clinic, Marshfield, Wis (Dr Madden); and Genentech Inc, South San Francisco, Calif (Dr Hamilton). Context Recombinant tissue-type plasminogen activator (rt-PA) improves outcomes for patients with acute ischemic stroke, but current approved use is limited to within 3 hours of symptom onset. This restricts the number of patients who can be treated, since most stroke patients present more than 3 hours after symptom onset.

A clinical study investigating the three months prognosis of patients with ischemic stroke treated with recombinant tissue plasminogen activator (rt-PA) and its effective factors

Objective: Currently, treating ischemic stroke by intravenous thrombolytic therapy has acceptable results in patients with stroke. This study aimed to evaluate the three months prognosis of patients treated with recombinant tissue plasminogen activator (rt-PA).Methods: This cross-sectional prospective study was conducted on 30 patients with cerebral ischemic stroke with the National Insti...

The Outcome of Treatment With Recombinant Tissue Plasminogen Activator in Acute Ischemic Stroke

Background: Thrombolytic therapy is the recommended treatment of acute ischemic stroke. It is crucial to evaluate the treatment results with recombinant Tissue Plasminogen Activator (r-TPA) in patients with acute stroke. Objectives: This study aimed to evaluate treatment outcomes with r-TPA in patients with acute stroke in a referral stroke center in Iran. Materials & Methods: In this retrosp...

Alteplase for acute ischemic stroke: outcomes by clinically important subgroups in the Third International Stroke Trial.

BACKGROUND AND PURPOSE Our aim was to identify whether particular subgroups of patients had an unacceptably high risk of symptomatic intracranial hemorrhage or low chance of benefit when treated with alteplase (recombinant tissue-type plasminogen activator). METHODS Third International Stroke Trial was an international randomized trial of the intravenous (IV) recombinant plasminogen activator...

Protocol for the perfusion and angiography imaging sub-study of the Third International Stroke Trial (IST-3) of alteplase treatment within six-hours of acute ischemic stroke.

RATIONALE Intravenous thrombolysis with recombinant tissue Plasminogen Activator improves outcomes in patients treated early after stroke but at the risk of causing intracranial hemorrhage. Restricting recombinant tissue Plasminogen Activator use to patients with evidence of still salvageable tissue, or with definite arterial occlusion, might help reduce risk, increase benefit and identify pati...
